The Nutritional Health Assessment consists of a 33 page questionnaire and review of your medical and dental records, which will help uncover the complex web of interactions of your history, physiology, and lifestyle that can lead to illness. Your unique genetic makeup is considered, along with both internal (mind, body, and spirit) and external (physical and social environment) factors that affect total functioning. 

Includes: 

         * The Nutritional Blood Analysis requires a copy of your most recent blood test,
